The first paper describing this treatment appeared in 1982: Miller, R.A., D.G. Maloney, R. Warnke, and R. Levy (1982). Treatment of B-cell lymphoma with monoclonal anti-idiotype antibody. New England Journal of Medicine 306(9):517-22.

Many additional papers have been published since then by Levy and colleagues, but the treatment has not been adopted, and the textbook authors continue to refer to it as "prohibitively expensive" without presenting any details of the cost, or comparison with the cost the now-standard eradication of all B cells (not just the lymphoma cells) with monoclonal antibodies to the protein CD20.
